Parent Financial Aid Forms

To submit these forms, please do one of the following:

  • Mail it to our office at 1001 Rogers St. Columbia, MO 65216
  • Deliver it by hand to your local campus

It can take 24 to 48 hours for your documents to be logged in once they have been received in the Financial Aid Office. Your student should visit eServices My Documents to verify that the documents have been marked received and to ensure other documents are not needed.
