Mid-term/Final Proctor Reservation Form - Rolla

Note: All fields (except Comments) are required.

IF YOU ARE RESERVING A SLOT FOR MFT TESTING:

  • Do NOT use this form!  Instead, send an Email to Steve Allen - spallen@ccis.edu - which includes your availability during the 5th and 7th Weeks of the session, your major, and which tests you need (MFT, MAPP, or both).

 

FOR MID-TERMS OR FINALS:

  • IMPORTANT: You are only allowed to take one exam per time slot. You may only reserve one test slot at a time using this form. If you wish to sign up for a mid-term and a final, you must fill out two separate requests.

  • PLEASE BE SURE to type in your Email address correctly: without it, we cannot email you with a confirmation of your reservation.
  • NOTE: The Email address will begin with your User Name and be followed by @cougars.ccis.edu
  • If you are unsure of your Course prefix, number, and section, and/or your Instructor's Name, that information can be found via CougarTrack under Class Schedule.
  • PLEASE verify the spelling!
